Clash Detection with Terrain Models

Does anyone have experience with doing clash detection with terrain models?  I'm a complete novice with clash detection, but I've watched the videos on the topic on the LEARN server.  I'm trying to use clash detection to verify that there is adequate cover over storm pipes, but the tool is not responding the way I'm expecting it to.  

For example, I know that there is one hard clash (where the pipe outlets into a detention pond).  When I run the clash detection, it detects a hard clash but the location provided isn't anywhere near where the clash is actually located at.  The location points to the center of the pipe run.

Also, I know that there are several areas where the cover on the pipe is 3 feet or less.  However, even when I put a clearance of 10 feet on the pipe, no soft clashes are detected.

Any help would be greatly appreciated!
